Physical Forms
Every student is required to have a physical on file in the Athletic Office dated on or after April 15th of the previous school year. Please read and fill out carefully. Some portions need to be completed by a physician, others by a parent/guardian. It is helpful to retain a copy for your own files. The copy may be submitted for subsequent sports as long as the date of the exam is still current.

Consent to Participate
A consent form is required for participation at the beginning of each sport. The MS form requires parent/guardian signature. The US form requires parent/guardian plus student signature.
